Dry Eye Disease: More Than Just Discomfort

Dry eye disease (DED) is one of the most common eye conditions in the United States — and one of the most underdiagnosed. It occurs when your eyes don’t produce enough tears, or when the tears your eyes produce evaporate too quickly, leaving the ocular surface unprotected and inflamed.

While dry eye is often dismissed as a minor nuisance, it can significantly impact quality of life, productivity, and vision quality. For patients considering LASIK or other eye surgery, dry eye must be evaluated and managed before and after any procedure.

Symptoms of Dry Eye

  • Stinging or burning sensation
  • Gritty or sandy feeling
  • Redness and irritation
  • Watery eyes (reflex tearing)
  • Blurred or fluctuating vision
  • Eye fatigue during reading or screens
  • Difficulty wearing contact lenses
  • Light sensitivity
Watery eyes can be a dry eye symptom. When eyes are too dry, they reflexively overproduce watery tears — but this reflex tearing doesn’t contain the oils and proteins needed to lubricate the eye properly. If your eyes tear frequently despite feeling irritated, you may have dry eye disease.

What Causes Dry Eye?

Dry eye has many potential causes, and most patients have multiple contributing factors:

Meibomian Gland Dysfunction (MGD)

The most common cause. Oil-producing glands in the eyelids become blocked, reducing the lipid layer of the tear film and causing rapid evaporation.

Age & Hormones

Tear production decreases naturally with age. Hormonal changes — particularly in menopause — significantly affect tear quality.

Screen Use & Environment

Reduced blink rate during screen use, dry air, wind, and air conditioning all accelerate tear evaporation.

Medications

Antihistamines, antidepressants, blood pressure medications, and many others can reduce tear production as a side effect.

Contact Lenses

Long-term contact lens wear disrupts the tear film and can damage surface cells, contributing to or worsening dry eye.

Autoimmune Conditions

Sjögren’s syndrome, rheumatoid arthritis, and lupus can cause severe aqueous-deficient dry eye.

Dry Eye Diagnosis at Carolina Vision Center

Effective dry eye treatment starts with accurate diagnosis. A one-size-fits-all approach rarely works, because the right treatment depends entirely on which component of the tear film is disrupted. Our evaluation includes:

  • Tear film osmolarity testing — measures concentration of the tear film, a key marker of DED severity
  • Meibography — imaging of the meibomian glands to assess gland structure and dropout
  • Schirmer’s test — measures aqueous tear production volume
  • Slit-lamp examination — evaluates corneal and conjunctival surface staining
  • Inflammatory marker testing (MMP-9) — identifies inflammatory dry eye

Dry Eye Treatment Options

Artificial Tears & Lubricating Drops

Preservative-free artificial tears are the first-line treatment for mild dry eye. Used consistently throughout the day, they supplement the natural tear film and reduce surface friction. Not all artificial tears are equal — our team will recommend the right formulation for your tear film type.

Prescription Anti-Inflammatory Drops

For moderate to severe dry eye, prescription drops such as cyclosporine (Restasis®, Cequa®) or lifitegrast (Xiidra®) address the underlying inflammation that contributes to tear deficiency. These medications require consistent use over several months to achieve full benefit.

Punctal Plugs

Tiny, dissolvable or semi-permanent plugs inserted into the tear drainage ducts of the eyelids to slow the drainage of tears — keeping more of your natural tears on the eye surface. A quick, in-office procedure with immediate effect.

Meibomian Gland Expression & Lid Therapy

For MGD-related dry eye, in-office thermal pulsation or manual expression of blocked meibomian glands can restore the lipid layer of the tear film. Combined with at-home warm compresses and lid hygiene, this can produce lasting improvement.

Omega-3 Supplementation

Dietary omega-3 fatty acids (fish oil) have been clinically shown to improve tear quality and reduce dry eye symptoms. Medical-grade formulations are more effective than standard over-the-counter supplements.

Corneal Conditions We Treat

Beyond dry eye, Carolina Vision Center diagnoses and manages a range of conditions affecting the cornea — the clear front surface of the eye:

Keratoconus

A progressive thinning and bulging of the cornea that causes distorted vision. Managed with specialty contact lenses; advanced cases may require corneal cross-linking or surgery.

Corneal Abrasion & Injury

Scratches to the corneal surface from contact lenses, foreign objects, or trauma. Prompt evaluation prevents infection and speeds healing.

Corneal Dystrophies

Inherited conditions (including Fuchs’ dystrophy) where corneal tissue gradually deteriorates, causing clouding and vision loss.

Recurrent Corneal Erosion

Repeated breakdowns of the corneal surface, causing sudden severe eye pain — often upon waking. Treatable with lubricants, bandage lenses, or laser therapy.

Frequently Asked Questions: Dry Eye & Corneal Disease

Can dry eye be cured permanently?

For most patients, dry eye is a chronic condition that is managed rather than cured. However, many patients achieve excellent long-term symptom control with the right combination of treatments — to the point where dry eye no longer significantly affects their daily life. In some cases, addressing an underlying cause (such as treating meibomian gland dysfunction or changing a medication) can produce lasting improvement.

Does dry eye disqualify me from LASIK?

Severe, untreated dry eye is a contraindication for LASIK, as LASIK can temporarily worsen dry eye symptoms. However, mild to moderate dry eye that is well-controlled does not necessarily disqualify you. Our pre-LASIK evaluation includes a thorough dry eye assessment, and we will help you manage dry eye before surgery if needed. In some cases, EVO ICL — which does not affect the cornea — is a better option for patients with significant dry eye.

Why do my eyes water if I have dry eye?

This is one of dry eye’s most confusing paradoxes. When the ocular surface is irritated and under-lubricated, the eye triggers a reflex response that floods the eye with watery tears. However, these reflex tears lack the oils and mucins needed for lasting lubrication, so they drain away quickly and leave the eye dry again. Treating the underlying dry eye — not just the watery eyes — is the solution.
